Preparing for Surgery at BID Milton

At BID Milton, we’re dedicated to ensuring a safe and smooth surgery process. The day before your procedure, you'll have a check-in appointment. During the appointment, you may need to have additional testing. You’ll also meet with members of our staff to go over the procedure and get answers to all your questions.

Please bring to your appointment:

• Driver's license or other photo identification.
• Any paperwork given to you by your physician's office, including doctor's orders or lab slips.
• A list of medications you take, including dosage and frequency.
• Your insurance cards and any other insurance-related information.
• Your advanced directive (health care proxy), if you have one.
