Outpatient Geriatric Consultation

To help you manage these conditions and others, we offer outpatient consultations with geriatrician Min Song, MD, MPH, CMD, HMDC, CPE. Dr. Song trained at Harvard and also studied public health, palliative care and hospice. He uses comprehensive geriatric assessment and works closely with our doctors to make sure they provide you with the most up-to-date geriatric care. This allows you to benefit from the expertise and resources of the Harvard Division of Aging. At the same time, you maintain your relationship with your own physician.

Care When You’re in the Hospital

The hospital can be a challenging place for anyone, especially seniors. We work to create easier hospital stays by making geriatric education a priority for our staff. What’s more, since 2007, we’ve partnered with Beth Israel Deaconess Medical Center’s Hospitalist program. It brings highly trained doctors to BID Milton to care for you while you’re in the hospital. They make sure you get the specialized attention you need. Geriatricians are also available to consult on complex age-related diseases.

After Your Hospital Stay

For your convenience, social workers and case managers help plan your discharge. They make sure you have the care you need when you leave the hospital and that your transition goes smoothly.

Outpatient Rehabilitation is available through BID Milton. If you need inpatient rehabilitation or long-term care, we’ve assembled a network of skilled nursing facilities. They offer high-quality gerontology services. We work closely with these facilities to help them meet your needs.
